  • Mr. [JOHN Klassen]'s death was caused or hastened by lousy care in a hospital. It is well known that medical mishaps happen daily in all hospitals, but Manitoba has been slow to get a handle on analyzing the extent of the problem here. Medical staff, particularly doctors, have resisted reporting mistakes that harm patients, which is mandatory by provincial legislation. The last WRHA report on medical error statistics was for 2004; a public report for 2005 is long past due. The authority's efforts to encourage reporting of errors have produced meagre results -- it is expected that some 2,000 injuries due to medical mishaps occur annually, but the 2004 report recorded only 174 "critical clinical occurrences," with 34 patients dying.

Looking at 10 areas of potential medical error, such as post-operative bleeding, collapsed lungs and injuries to newborns.
