Summary
Note that declarer's play was foolproof so long as West held the queen of diamonds since, even if West was able to produce a second spade, declarer could insert either the queen or nine from dummy, ruff out East's king and later take a heart discard on a spade winner.
Also, had East produced the queen of diamonds, declarer could ruff and exit in hearts. This would afford the additional chance that, after West took his two heart winners, that his spade lead had, in fact, been a singleton, and at that point he would be obliged to play a red card and thereby obviate declarer's spade loser.See the full content of this document
